If medications are brought to the camp, it is required they are in the original container, with camper's name and instructions printed clearly. Almost all medications will be handed over to the designated nurse. At the discretion of the Camp Nurse and guardian, some medications may be left with the youth.

In the event of an emergency, do you, the parent(s)/guardian(s) of the camper:

1. Give your consent to any examination, medical or surgical diagnosis rendered under the general or specific supervision of any licensed medical or emergency room staff under the provisions of the Medical Practice Act and on the staff of any acute general hospital holding a current license
   
   
2. Give your consent to any examination, medical, or surgical diagnosis rendered under the general or specific supervision of any licensed dentist under the provisions of the Dental Practice Act.  
   
   
3. Give authorization in advance of any specific diagnosis, treatment, or hospital care being required and give authority and power to render care which the physician above in the exercise of his/her best judgment may deem advisable.  
   
   
4. Understand that effort shall be made to contact the undersigned before rendering treatment to the patient but that any of the above treatment will not be withheld if the undersigned cannot be reached.
